How much will my insurance cover?

For specific details about costs associated with your appointment, we recommend contacting your insurance directly to confirm:

  • Your medical benefits, including whether your plan covers your appointment type, and if there are any excluded benefits (e.g. video wellness exams)

  • Any out-of-pocket costs associated with your appointment (including potential deductibles, copays, and coinsurance)

  • If a specific provider is in-network with your insurance plan

How do I confirm my provider is in-network?

Contact your insurance

Call the Member Services or Customer Service number typically on the back of your insurance card. Be ready to share your member ID, full name, and date of birth.

Provide our practice details (NPI, Tax ID)

Since we operate in multiple states and work with various insurance networks, your insurance may also need the details of the specific Circle Medical practice that will bill your visit to determine if our practice is in-network.

Confirm your individual provider is in-network

If you’re booking—or already booked—with a specific provider, we recommend confirming that this provider is in-network with your plan. While our practice may participate in your insurance’s network, not every provider may be in-network with your insurance plan.
